to progress the story, find where the cave key goes (spoilers, so i wont tell you where it goes)
if you did beat THOTK, then you have to use the BC you got to get the next one from him, which is a 1BC run
adding a BC to your runs makes them harder, but gives you more rewards and progresses the story
