My guess is that no, he won't have any identity problems as he knows who he is and this effect will not have any long term effects as long as he has his fairies and their magic.

As normal people don't have this it effects their lives even more especially during transition but Timmy doesn't have that part in and it's not as if this would last long.

It isn't boring, it's building off of the plot of the original comic. It isn't just porn, it's an actual story, and honestly it kindve feels solely like an immersive story rather than anything else.
